Ashley Smalley Nutrition, LLC is in-network with 5 insurance carriers:
Anthem/Blue Cross Blue Shield of Maine
Harvard Pilgrim Healthcare

United Healthcare
Cigna
If you do not have one of these 5 insurance providers, then you can request a Superbill (i.e. an itemized invoice) that you can submit to your insurance company directly to seek reimbursement.

Nutrition counseling services at Ashley Smalley Nutrition, LLC are often covered as a preventive service. Please call the number on the back of your insurance card and ask the following questions to verify the insurance benefits you have for nutrition counseling. This will help ensure you get the most out of your benefits and avoid unexpected costs.
Questions to ask your insurance carrier:
Is Ashley Smalley/Ashley Smalley Nutrition (NPI# 1487080198) a covered provider under my plan?
Do I have benefits for nutritional counseling (ask for procedure codes 97802 & 97803)?
Is code z71.3 (dietary counseling and surveillance) covered as a primary diagnosis, or is a medical diagnosis required*?
*If a medical diagnosis is required, simply have your doctor fax recent visit notes and your diagnosis list to me at 207-544-5136.
Do I have any nutrition counseling benefits covered under the preventative care portion of my plan? (diagnosis code z71.3)
Is a doctor’s referral or order required (Harvard Pilgrim plans typically require one with a medical diagnosis code)?
Are nutrition services provided via telehealth covered (and if so, is there a copay)?
Is there a limit on the number of visits allowed? (ask about in-person visits AND telehealth visits).
Do I have a deductible to meet before insurance pays (if not under preventative care)? If yes, what is it?
Do I have a co-pay or co-insurance? If yes, what is it?
Ask for the reference number of your call and the date.
Although Ashley Smalley Nutrition may be able to assist you in this process, it’s your financial responsibility to know what your plan covers and any co-payments you may have before our sessions.
Please note, in the event that Ashley Smalley Nutrition does not accept your insurance or is considered out-of-network, you will need to pay for your counseling session at the time of service. Ashley Smalley is happy to provide you with a superbill (i.e. an itemized invoice) that you can submit to your insurance company directly for reimbursement.
Payment:
Cash, check, credit/debit and HSA/FSA cards are all accepted. Please note, a credit/debit card on file is required for all appointments.
